The mechanism is straightforward: California Governor Gavin Newsom and the Legislature expanded full-scope Medi-Cal to all undocumented adults in 2022, regardless of immigration status. Multiply that across even a modest number of patients in San Francisco’s shelters alone, and the costs compound quickly. According to the Budget Committee, the state is already on track to spend $8.4 billion this fiscal year on Medi-Cal health benefits for illegal immigrants.
